I can see from your screen shot that your submit button is greyed out, this occurs when you have the "Disable on form invalid" option checked. I recommend disabling this using the steps below:
Open the component settings for the submit button
Under the display tab untick the "Disable on form invalid"
Now users can click submit and the form will highlight any issues
As for the approval section, it looks like you have used one of our templates without updating the permissions to specify which Microsoft 365 users are allowed to fill in the approvals section. I recommend watching out Youtube video on this topic (shown below).

Just closing this issue as it has been resolved offline. For any future readers, the issue was the configuration of the email notifications. The author had been sending the View URL instead of the Edit URL. This led to users opening a URL that launched the response in read-only mode.
