Hi Tom,
The likely reason there is no follow-up email is because of the conditions you have set up in the email recipients (to:) field. I recommend checking that there is a conditional block added to cover who to send an email to once the signature component has data (i.e. is signed).
In case it helps, here is an example of an email configuration which will send an email to the original responder once the request has been approved.
